PREVENT is race-free and estimates your 10-year risk of a first hard atherosclerotic cardiovascular disease (ASCVD) event (a heart attack or stroke) from age, cholesterol, blood pressure, diabetes, smoking, BMI, kidney function (eGFR), and statin use. It was published in Circulation in 2024 (Khan et al.). Underneath the PREVENT result, the calculator also shows the legacy 2013 ACC/AHA Pooled Cohort Equations (Goff et al., Circulation 2014) as a clearly labeled comparison figure, since PREVENT typically reports a lower percentage than the older model for the same person and a reader may have been given a PCE-based number by a clinician.

No. It produces a population-based statistical probability, not a diagnosis. It estimates risk of a first hard ASCVD event only, and does not apply if you already have cardiovascular disease, LDL cholesterol of 190 mg/dL or higher, or diabetes with additional risk factors — those groups are already considered candidates for statin therapy regardless of score. Always discuss your result with a healthcare professional.

Per the 2026 ACC/AHA/multisociety dyslipidemia guideline's PREVENT-based bands (the headline result on this page), under 3% is Low risk, 3% to under 5% is Borderline, 5% to under 10% is Intermediate, and 10% or higher is High; LDL-lowering therapy is generally recommended at Intermediate and definitely recommended at High. The legacy 2013 Pooled Cohort Equations comparison figure uses different, higher thresholds from the 2018 cholesterol guideline instead — under 5% Low, 5% to under 7.5% Borderline, 7.5% to under 20% Intermediate, 20%+ High. The two sets of bands are not interchangeable: match each percentage to its own model's bands rather than comparing the raw numbers.

The headline PREVENT result is validated for ages 30-79, BMI 18.5-39.9, and eGFR 15-140, and is race-free by design; results outside those ranges are indicative only. The legacy Pooled Cohort Equations comparison figure shown alongside it is validated for ages 40-79 only and provides coefficients for just two race categories — White and African American — so other races and ethnicities are approximated there using the White coefficients, a coarse approximation. Both models were derived and validated primarily in US populations, so accuracy may vary elsewhere.
